Inner Peace & Harmony provides outpatient, community-based case management and care coordination for individuals across the lifespan, including:
Assessing needs and developing individualized plans of care
Coordinating mental health, medical, and community services
Connecting individuals to housing and basic needs through care coordination and resource linkage
Supporting safety planning and care transitions
Ensuring follow-up and continuity across systems

Support includes screening for housing instability and unmet basic needs, coordination of referrals to appropriate housing programs and community resources, assistance with applications and documentation, and follow-up to support stability.
Inner Peace & Harmony does not provide housing or direct financial assistance.

Trauma-informed coordination and safety planning for individuals impacted by domestic violence, including connection to appropriate community and protective resources.

Short-term coordination during periods of instability, including linkage to crisis services and support during transitions from inpatient, residential, or higher levels of care.

Coordination of services and supports following hospitalization, treatment programs, or crisis interventions to promote continuity and reduce gaps in care.
