Support includes screening for housing instability and unmet basic needs, coordination of referrals to appropriate housing programs and community resources, assistance with applications and documentation, and follow-up to support stability.

Short-term coordination during periods of instability, including linkage to crisis services and support during transitions from inpatient, residential, or higher levels of care.

Coordination of services and supports following hospitalization, treatment programs, or crisis interventions to promote continuity and reduce gaps in care.
