Inner Peace & Harmony, LLC is a nurse-founded, nurse-led outpatient case management and care coordination organization supporting children, adults, and families across the community.
We coordinate care for individuals with a wide range of needs, with a specialized focus on connecting individuals living with mental illness to comprehensive community-based services—while remaining accessible to those facing other care gaps, barriers, or complex life challenges.
Our approach is grounded in the nursing process, emphasizing assessment, individualized care planning, coordination, and consistent follow-through to promote stability, continuity, and long-term outcomes.
What We Do
Inner Peace & Harmony provides outpatient case management and care coordination through a nursing-led, team-based model.
